APPROACH TO LOW BACK PAIN

Low back pain is the most common cause of limitation of activity in patients younger than 45 years, the second most common cause of the visits to primary care physician, the fifth cause of hospital admissions, and the third most common cause of surgical procedures. It is estimated that the 75 % to 80 % of the adult population will have low back pain once in a lifetime. The majority of the patients will not seek medical care and will get better after a short term of treatment. Nevertheless, among 10 % of these patients will develop chronic persistent pain or recurrent pain. In order to classify the origin of low back pain, a complete medical history and physical exam must be undertaken. Image studies should be ordered in a judicious manner based in a symptom directed algorithm. Among others, simple X-rays, computed axial tomography, and magnetic resonance should be considered. Electromyography is also an important diagnostic tool. Once conservative measures have failed and the patient continues to persist with low back pain that limits normal day activities for more than 6 months, with or without radicular symptoms, surgical options must be considered.
